Improve patient outcomes and capture lost revenue by automating the identification and follow-up of incidental findings across complex health system EMR data.
Education Requirements:
Bachelor’s degree in Nursing is required
KS or MO nursing license required
ability to obtain and maintain multistate license and/or single state licenses in various states as needed
Experience Requirements:
5 years acute care nursing experience required
Other Requirements:
Excellent written and verbal communication skills
Excellent customer service skills
Computer literacy required
Leadership experience preferred
Specialty nursing certification preferred
Travel requirements are approximately 4-8x per year
Responsibilities:
Performs quality review of patients identified by our software including but not limited to patient chart reviews
Facilitates the ongoing coordination of care for patients with incidental findings
Responsible for notifying either the patient or his/her provider when disease state is off surveillance protocol
Accurately document all aspects of care coordination and follow up in patient management system
Establish and maintain positive relationships with all involved providers
